Before the Christmas recess, it was revealed a damning report from a consultant midwife into maternity services at the Cwm Taf health board wasn’t shared by senior managers at the end of 2018.
Maternity services at the health board – subsequently reorganised into Cwm Taf Morgannwg health board – were placed into special measures in April 2019 after the report revealed a catalogue of failures in patient safety and complaints handling.
It’s understood that the report wasn’t shared because it was deemed to be a “draft”.
